P2415Q, "DisplayPort Link Failure" when display comes back from power save

Video Card: AMD Radeon R9 290x (Sapphire Tri-X R9 290X 4GB GDDR5 OC(UEFI), New Edition)
Card Driver Version: AMD Catalyst 14.12
Video Out Ports Available: 2x dual link DVI-D, 1x HDMI, 1x DisplayPort 1.2
Used Port: tried both card DisplayPort to monitor mini DisplayPort and card DisplayPort to monitor DisplayPort
Cable: tried both DP to mDP cable included with monitor and Accell UltraAV DP to DP cable, part number B088C-007B (on DisplayPort's list of certified cables)
Monitor OSD shows Resolution: 3840x2160, 60Hz
Monitor purchased from Dell

In general, everything works fine--I get the full 3840x2160 60Hz that I'm expecting, no problems there. However, in the Windows Power Options, I have it set to "Turn off the display: 10 minutes". After 10 minutes of idle time, Windows turns off the display and the monitor goes into power saving mode, as expected. The problem is when I move the mouse to turn the monitor back on, it usually comes back on with no apparent problems, but on the screen is an error message popup that says:

DisplayPort™ Link Failure

The system has detected a link failure and cannot set the requested resolution and refresh rate on your DisplayPort™ display. Your display might not support the requested resolution or there may be an issue with the cable connecting the display to your computer.

Of course, it's difficult for me to tell whether the error pops up when Windows turns the monitor off or whether it pops up as it's turning the monitor back on. Apparently it's the AMD Catalyst Control Center that's displaying the message. I don't get the message 100% of the time, but I'd say I get it about 80% of time. As I said, I don't notice any problems when it turns back on... in particular, the resolution and refresh rate are fine; it's still 3840x2160 60Hz. If this were the only problem, I'd just check the "Don't display this message again" box and be done with it.

However, normally run a dual monitor setup--the above was all tested with only a single monitor plugged in to show that the "DisplayPort Link Failure" isn't related to dual monitors. But with dual monitors, the other issue that I see is that any windows I had on the secondary monitor (Dell UltraSharp 2408WFP connected via DVI) are moved back to the primary P2415Q when the monitors come back on. This doesn't happen if I don't get the DisplayPort Link Failure error, so it seems that they're related.
